Answer:
The most common way to exploit weak play is to very often raise pre-flop to take control of the pot. Often you can isolate a weak limper this way and take the pot on the flop. If you get called on the flop without hitting, you can take the free card on the turn if you are in position.
To decide whether the bet is a scared bet or a value bet depends on the player of course. One rule that applies to most weak opponents is that a minimum bet without the lead is a sign of weakness. If you don't have the lead in the pot you often need to call on the flop if you hit any pair and you are heads-up, because most players make continuation bets if they raised pre-flop. There are of course exceptions. If, for example, a very tight player bets you can fold anything lower than top pair.
It is pretty rare for people to continue the bluff on the turn. It takes much more strength to fire a bullet on the turn, and most people don't have it in them.
The ability to play from out of position can be important but it is hard to do. Do yourself a favor and throw away anything but strong hands if you are out of position. It is a much easier way of playing poker and you won't face as many difficult situations that can put you of your game.
